  1. Primary / Clinical Health Care

The role is required to provide primary health care for patients by examining, counseling, and treating the patient.


  1. Care Planning and Case Management

Participate in the development and implementation of case management and care coordination into service delivery within the program.


  1. Public and Environmental Health

The role works within a population health approach through a primary health care context, using quality improvement principles, processes, and tools. Supporting existing and new health programs within the Njernda Aboriginal Corporation.

Perform a range of general duties as directed in relation to the position.


  1. Accreditation work within RACGP Guidelines

Working within the Njernda Aboriginal Corporation accreditation policies and procedures.

  1. Primary and Clinical Health Care
  • Provide primary health care services to clients
  • Development and on-going management of patients including Care Plans (GP Management plans) for clients diagnosed or identified as at risk of developing Chronic Disease.
  • Provide Care Planning services and management to other Njernda Aboriginal Corporation programs and services as required
  • Involve Aboriginal Health Workers with all forms of client care through education and support wherever possible.
  • Authorisation and interpretation of pathology results
  • Promote and conduct Health Assessments
  • Notification of notifiable diseases
  • Provide patient and staff Health Education as required
  • Where required attend a home visit and provide off site clinics and health services
  • Assessment and referral of social welfare issues
  • In line with the Njernda Aboriginal Corporation requirements maintain patient medical records


  1. Care Planning and Case Management
  • In conjunction with relevant health professionals and Aboriginal Health Workers monitor care plans
  • Actively organise and participate in client case management sessions where required
  • Refer patients to care plan reviews as required
  • Liaise and refer to Njernda Aboriginal Corporation programs and external agencies when required


  1. Public and Environmental health
  • Working within a Population Health context proactively assist in the identification, management, and support through the development of policies and procedures that tackle the major health issues facing the local Aboriginal community that Njernda Aboriginal Corporation services. including but not limited to.

  • Awareness and support of the various health programs conducted at the Njernda Aboriginal Corporation, including:


  • Family Support Services
  • Baroona
  • Yakapna
  • Berrimba Child Care Centre
  • Best Start
  • HACC
  • Emotional and Spiritual Wellbeing Program
  • Drug & Alcohol Programs



  • To collect, document and utilize patient data through the use of the Communicare Software
  • Maintain documentation and client records for clients presenting to the Medical Clinic programs in accordance with the Njernda Aboriginal Corporation policy
  • Ensure there is a transfer of professional skills and knowledge to Aboriginal Health Workers in the organisation
  • Participate in the delivery of structured training and support for students and health workers at the Njernda Aboriginal Corporation where required.
  • To understand and promote the philosophy of the Njernda Aboriginal Corporation
  • Undertake other duties, which are incidental and peripheral to the main tasks, provided that such duties are reasonable within the employee’s competence and training



  • Adhere to the Njernda Aboriginal Corporation accreditation program and processes



  • Observe the safe working practices you have been trained in, as far as you are able, protect your own and others’ health and safety.
  • Participate in on-going training when necessary, including registrar training requirements and/or CPD points where applicable.
  • Participate in general staff and unit meetings
  • Act at all times in a professional manner in accordance with the policies and practices of the Njernda Aboriginal Corporation
  • All Njernda Aboriginal Corporation employees must comply with the code of conduct as articulated in the Njernda Aboriginal Corporation Policies and Procedures
  • To provide continuity of care between Practitioners and other medical staff.




Experience and Qualifications in:


  • Understanding of and Commitment to the Aboriginal community.
  • A qualified medical practitioner registered to practice in Victoria. Registration must be presented to HR annually.
  • Current Medical Indemnity Insurance. Insurance must be presented to HR annually.
  • Current provider and prescriber numbers
  • Good computer and telephone skills,
  • Knowledge of Health Promotion strategies
  • A demonstrated understanding of working in a community-controlled health service
  • The demonstrated ability to work effectively as part of a multi-skilled team
  • The capacity to participate on weekend rosters.
  • Knowledge and commitment to occupational health & safety legislation
  • Good written and verbal communication skills and the ability to communicate effectively with patients, carers, and the Aboriginal communit

  • Appointment is subject to a satisfactory Police check & Working with Children check.
