Key Responsibilities
- Provide client centred care to each young person registered in the Youth Program, through:
- conducting an assessment of each young person’s needs when they join the program developing a goal driven care plan for each young person on the program
- regularly monitoring the progress of each young person’s goals o providing one-on-one support and mentoring to each individual young person using a case management approach based on each individual young person
- making referrals to other supports and services, as required o linking young people into relevant programs, such as the L2P, homework classes,
- making recommendations for use of the Youth Program’s Brokerage Funding to support young people to participate in organised sport activities, as required.
- Coordinate group-based activities for young people focusing on healthy lifestyles.
- Track the impact of the program on the health and wellbeing of each young person.
- Build and maintain partnerships with the local secondary schools, sporting clubs (including the Warma Sports Club), training and education providers and employment services in order to support young people achieve their goals.
- Maintain appropriate data and program and client records, and provide quarterly reports to the Youth, Justice & Residential Services Manager
- Work as a team member, and work closely with the Youth Services Team Leader & Engagement Workers, Community & Youth Justice, Health Promotion Team, SEWB
Team, Family Services Team, Berrimba Childcare & Early Years Services, Health and Administrative staff
- Support other team members in their work.
- Work cooperatively to engage young people in local community events e.g. family days, NAIDOC week, health days etc.
- Attend and participate in team meetings, general staff meetings, etc.
- Represent the organization and/or community in forums outside the agency
- Participate in ongoing training and development.
- Ensure the confidentiality of client information at all times.
- In consultation with the CEO, monitor and evaluate program delivery.
- Other duties as directed.
- Act, at all times, in a professional manner in accordance with the policies and practices of the corporation.
Key selection criteria
- A proven knowledge and understanding of aboriginal people and aboriginal culture.
- Demonstrated knowledge and understanding of issues relating to the needs of at-risk aboriginal young people in the area.
- An ability to communicate effectively with aboriginal young people and advocate on their behalf .
- Proven capacity to establish effective working relationships with a wide range of services and professionals.
- Demonstrated ability to provide activities and supports that genuinely interest and engage young people, empowering them to make positive choices
- Sound negotiation skills.
- Demonstrated experience facilitating group work and case work with young people
- Good written and verbal communication skills.
- The ability to maintain records and manage time effectively and efficiently.
- Demonstrated ability to work cooperatively and effectively as part of a team.
- Computer skills.
Other relevant information
- The successful applicant will be required to undergo satisfactory pre-employment checks, including providing at least two professional referees, completion of a medical declaration, Police Records Check, Working With Children Check, Victorian Driver’s License, and proof of identify and qualifications
