Njernda Aboriginal Corporation is seeking a HACC-PYP / CHSP Worker to deliver support to clients under both the Home and Community Care Program for Younger People (HACC PYP), for Aboriginal and Torres Strait Islander individuals under 50 who are at risk and require assistance to live independently, and the Commonwealth Home Support Programme (CHSP), for those aged 50 and over. You'll help younger people with a disability, illness or care needs stay connected to community, and support older people to remain independent and safe at home.

About the role

Reporting to the Health and Ageing Manager, you'll work closely with the Health and Ageing team, HACC PYP and CHSP providers, My Aged Care, NDIS Local Area Coordinators and community health services.

• Deliver direct client support services including social support, domestic assistance, transport and planned activities, applying a strengths-based and reablement approach
• Coordinate and monitor service delivery, manage client waitlists and priority lists, and contribute to meeting agreed service hour targets
• Complete intake assessments, goal-directed care plans, reviews and incident reports, with reassessments undertaken annually or when circumstances change
• Identify clients with complex needs and facilitate timely referrals and warm handovers to appropriate programs
• Provide culturally safe, trauma-informed and person-centred support to Aboriginal and Torres Strait Islander clients and their families
• Ensure Aboriginal clients are actively prioritised in access decisions as a designated priority group under HACC PYP and CHSP
• Maintain accurate, contemporaneous case notes and enter client data daily

Selection Criteria - these must be addressed

To be considered, your application must include a separate document responding to each of the numbered criteria below.

1. A Certificate IV in Community Services, Individual Support, Aged Care with relevant experience, or equivalent experience or study
2. A minimum of 3 years' experience in a similar community services, aged care or disability support role
3. Proven knowledge and understanding of Aboriginal and Torres Strait Islander peoples, culture, community and the social determinants of Aboriginal health, essential for this role
4. Demonstrated ability to communicate effectively and sensitively with Aboriginal community members and handle complex, sensitive matters appropriately
5. Knowledge of HACC PYP guidelines and regulations, including eligibility criteria, priority of access, program boundaries and reporting requirements
6. Understanding of the CHSP and the Aged Care Act 2024, including the rights-based framework for older clients
7. Strong assessment, care planning and service coordination skills
8. Intermediate computer skills, including data entry, reporting, Microsoft Word and Excel
9. Ability to maintain accurate records, manage time effectively and prioritise competing demands
10. Demonstrated ability to work cooperatively and effectively as part of a multidisciplinary team
11. Experience with the My Aged Care Service and Support Portal
12. Knowledge of HACC PYP/CHSP Linkages case management processes

Desirable: Aboriginal and/or Torres Strait Islander descent, a Diploma in Community Services, Aged Care, Case Management or a related field, and knowledge of HACC/CHSP and other aged care or disability programs and transition pathways.
