A chronic condition is a health issue that lasts (or is likely to last) six months or more. This can include:
- Asthma
- Diabetes
- Heart disease
- Stroke
- Cancer
- Musculoskeletal conditions (like arthritis)
Our team offers chronic disease management plans and preventative care to help keep you well and living a full life. We work alongside you to monitor your condition and connect you with other supports if needed.
If you’re eligible, you can access these services at no cost through our clinic. Referrals can be made through a Njernda GP or any other General Practitioner.
