For Aboriginal and Torres Strait Islander people under 50 who need a hand at home
Our HACC PYP program offers practical, entry-level support to help younger Aboriginal people when their capacity for independent living is at risk.
This may be due to chronic illness, mental health issues, disability or other conditions where they need one-off, intermittent or ongoing support to undertake the activities of daily living to stay well and independent at home. This includes help with daily tasks, access to health services, and short-term care after an illness or hospital stay.
You may be eligible if:
- You are Aboriginal or Torres Strait Islander and under 50 years old
- You are not already receiving an aged care or NDIS support package
- You need support with regular activities to live safely and independently at home
Support may include:
- Personal care (e.g. help with dressing or shopping)
- Meals and nutrition
- Transport
- Help around the house (cleaning, gardening, small maintenance)
- Access to equipment like walking frames
- Social activities and respite care
- Home modifications (rails, ramps)
- Nursing, physio and other care services
Please note: Njernda may not offer all services directly, but our staff will help you access the supports and services you need.

Referrals can be made by your doctor, Njernda’s Medical team, a hospital worker or one of our HACC PYP staff.
